چکیده
PATIENT Male, 23 FINAL DIAGNOSIS: Sinus of valsalva aneurysm (SVA) Symptoms: Chest pain • low O2 saturation • short of breath MEDICATION - Clinical Procedure: - Specialty: Cardiology. OBJECTIVE Rare disease. BACKGROUND Sinus of valsalva aneurysm (SVA) is a rare cardiac anomaly. It may be congenital or acquired; a coexisting cardiac lesion might be present. Rupture of the aneurysm, where it usually occurs in the right atrium, can cause acute symptoms of heart failure. Echocardiography (particularly TEE) can provide all necessary diagnostic data for safe surgical treatment. Treatment of choice is surgery. CASE REPORT A 23-year-old male, previously healthy, presented to the emergency room (ER) with shortness of breath for the last 10 hours after lifting a heavy object. The patient had central chest pain. His O2 sat was 88%. ECG showed ischemic changes. Diagnosis of AMI was made, but auscultation revealed a murmur followed by a TTE and TEE, which revealed a ruptured sinus of valsalva aneurysm. CONCLUSIONS This case report highlights the superiority of TEE over TTE in diagnosis and in planning adequate surgical treatment for patients with ruptured SVA, as well as the importance of ultrasonographer experience in the diagnosis. The optimal management for a ruptured SVA is surgical repair, with an acceptably low operative risk and good long-term outcome.

A rare cause of sudden dyspnea and unexpected death in adolescence: fistula from aortic sinus of Valsalva to right atrium.
Aneurysm of the sinus of Valsalva is a rare cardiac abnormality. Unruptured sinus of Valsalva aneurysm is usually asymptomatic, however, a ruptured aneurysm typically leads to an aortocardiac shunt and progressively worsening heart failure. We report a case of a 17-year-old male who suffered an aneurysm of the sinus of Valsalva rupture into the right atrium who manifested sudden dyspnea and acu...

We describe the successful surgical repair of an acute aortic dissection that had caused an aorto-right atrial fistula in a 67-year-old man. The patient was admitted to the hospital on an emergency basis because of severe heart failure. The diagnosis of acute aortic dissection with rupture into the right atrium was confirmed by use of intraoperative transesophageal echocardiography, although ru...

A 61-year-old man was admitted to the hospital because of orthopnea and was diagnosed with heart failure with a continuous heart murmur. A transthoracic echocardiogram revealed turbulent flow from the right coronary sinus of Valsalva to the right atrium throughout the cardiac cycle. Aortography confirmed the presence of a shunt jet from the right coronary sinus of Valsalva to the right atrium. ...

Ruptured noncoronary sinus of valsalva aneurysm with fistulous connections to multiple cardiac chambers has not been reported previously. We report a 66-year-old man who presented with worsening cough and exertional dyspnea. Transesophageal echocardiogram confirmed a large aneurysm involving the noncoronary cusp of the aortic sinus with aneurysmal extension to the left atrium. There were also t...

Sinus of Valsalva aneurysm accounts for only 1% of congenital cardiac anomalies. Sinus of Valsalva aneurysm can cause aortic insufficiency, coronary artery flow compromise, cardiac arrhythmia, or aneurysm rupture. A 44year-old male presented with a 3-month history of palpitation and exertional dyspnea. Cardiac auscultation revealed a grade III/VI ‘‘to and fro’’ murmur along the left sternal bor...
